Immigration Forms

Please go to the New Zealand Immigration website to download copies of New Zealand immigration forms.

Electronic online immigration applications – please go to www.nzimmigrationmedical.co.nz

Please complete the New Zealand immigration form to question B30 on page 5 but do not sign the form as your signature needs to be witnessed by the examining doctor. 

Please bring with you to your appointment

• Forms
• Passport
• Passport Photos - 3 photos are required
• Glasses – please bring your glasses, or contact lenses for the eye testing

You need to arrive at least 15 minutes before the appointment time so that reception can check that your forms are in order before the medical examination begins.

Please note that a urine sample is required to be done at CityMed for the medical (Women – please note that the medical cannot be completed during your period because the urine sample cannot be done during menstruation or within 3 days of it finishing.)

Standard Adult Fee $270.00 (permanent - including x-ray + HIV test ) or $250.00 (temporary - including x-ray)

(Note: Fees are subject to actual medical requirements as additional tests maybe required e.g. for diabetes).  

Courier fees - an additional fee of $20.00 applies if the medical is being forwarded to Sydney, Australia.

Please complete the Australian immigration form Part A to question 18 and enter your passport number in the box on the top of every page. Do not sign the form as your signature needs to be witnessed by the examining doctor.

• Proof of immunisations that you have had, if possible.
