CityMed is a modern primary healthcare facility, which offers you a full range of integrated medical services in the heart of the Auckland City. This site will allow you to visit CityMed and check out our facilities, all the services that we provide and the team that provides them. 

You can print an enrollment form from here if you are a new patient and wish to become funded at CityMed.  (If you are a non New Zealand resident, on a student visa or any other visa of less than 2 years you are not eligible).  You will then need to complete all sections, sign it and bring it with you to your first appointment.  Patients that enroll from the web will receive $10.00 off their first consultation at CityMed and also reduced prescription fees.
